Help Tell the Story of How Covid-10 Has Affected You and Your Community

The Yale Cancer Center and the Yale School of Public Health are seeking New Haven residents (English and Spanish speakers) to inform research on how Covid-19 has affected this city’s residents. This work is being coordinated by Cancer Action Prevention & Engagement. Why does your participation matter?
1.You’ll be contributing to research that strives to get the fullest picture of Covid-19’s impact on all of New Haven’s communities
2.You’ll contribution to a database that will contribute to better health policy and healthcare practice.

Q: What is it?
An online survey (through a computer, tablet, or a smart phone) about your experiences during the COVID-19 pandemic and other issues related to your health.
Q: How long will it take?
The survey will take about 30 minutes, but you will have an hour once you begin. PLEASE complete the survey at one time. Contact us if you have a problem along the way.
Q: What if I need help filling out the survey?
No problem! Call 203-200-3030. Leave a detailed message in Spanish OR English. We will call you back to help you complete the survey.
Q: How will I receive my $25?
Once you complete the survey, you will be provided a link within to access a digital gift card for Amazon, Visa, or Walmart.
