CARE Community Engagement and Project Coordinator
We are looking for a community organizer with a passion for improving health in New Haven. The Community Engagement and Project Coordinator will support our community-based programs and outreach and engagement work. 
The Community Engagement and Project Coordinator is responsible for:
  • Building relationships with New Haven residents, local organizations, and municipal departments for successful partnerships on projects and research initiatives that address health in New Haven. 
  • Community organizing with residents to develop health-related agendas and initiatives
  • Supporting CARE’s Health Leaders programs by identifying and training leaders on health and the social determinants of health and contributing curriculum for training, while assisting in the facilitation of training sessions. 
  • Working with various community-based organizations to plan, facilitate, and participate in community-level work, including assisting with the execution and coordination of program and policy level interventions.

REACH Project Manager
We are looking for a detail-oriented candidate with a passion for improving health in New Haven. This position serves as the project manager for the Racial and Ethnic Approaches to Community Health (REACH) grant, supported by the Center for Disease Control (CDC). The REACH Project Manager will assist the Director of CARE in the administration, coordination, and implementation of grant-related goals and objectives. This person also is responsible for supporting the day-to-day operation and coordination of the activities associated with the grant.

The Project Manager is responsible for:
  • Collaborating with the Director on administration and delivery of the grant program and all related activities, including but not limited to managing deliverables to the CDC. 
  • Assists in the design and implementation of grant-related programs, services, and training. 
  • Works with staff and subcontractors to monitor CDC contract compliance and implement activities. Responsible for tracking required reporting and ensuring deadlines are met.
  • Works closely with the Director and subcontractors to gather appropriate information, create systems for collecting information, and synthesize information for reporting. Assists in the preparation and submission of federal data reporting requirements. Assisting the Director in ensuring all data is inputted into the database for the CDC.
  • Attends and participates in REACH coalition and working group meetings.
