Cornell Scott–Hill Health Center Announces New Board President; Celebrates Recent Innovations And Accomplishments During National Health Center Week
New Haven, CT. — The Cornell Scott–Hill Health Center, a leading provider of community healthcare for over forty-five years, is pleased to share that August 7th to the 13th is National Health Center Week. National Health Center Week occurs every August and celebrates the work and services Health Centers provide to the communities they serve. This year’s theme is "Celebrating America's Health Centers: Innovators in Community Health." In keeping with that theme, the Cornell Scott-Health Center is celebrating its recent innovations and accomplishments.
“There are so many amazing things going on at the Cornell Scott-Hill Health Center,” said Michael Taylor, the Health Center’s CEO. “From innovative programs to expanded services, this is an exciting time for not only for the Health Center, but more importantly for the people we serve.”
Here’s a look at what’s happening:
· Angel Fernandez-Chavero: The Health Center is pleased to announce that its Board of Directors has elected Angel Fernandez-Chavero to serve as its President. Fernandez-Chavero first joined the Board in May 2013. He was elected 1st Vice President in June 2014, and reelected to that position in June 2015. He has also served on the Center’s Finance and Governance Committees. Fernández-Chavero is the Managing Director and founder of Aspire Praxis, LLC, a firm that offers consulting services to nonprofit organizations including strategic planning, development, and management assistance, as well as philanthropic advice regarding major gifts, planned giving and endowment development.
· Medication Therapy Management (MTM) Program: The Health Center is a proud recipient of a $194,500 grant from the Connecticut Department of Social Services. The grant, administered by Community Health Network of Connecticut, will enable us to develop and implement a pilot, community-based MTM program, where our pharmacists will work as integrated members of a patient’s health care team. Patients who qualify will meet one-on-one with our pharmacists, who will assess the patients’ medication and supplement usage and work with them on building the skills needed for effective self-care.
· Medication-Assisted Treatment (MAT) Program: The Health Center is also the recipient of a $325,000 two year grant from the U.S. Department of Health and Human Service’s Health Resources and Services Administration. With Substance Abuse Service Expansion Award funding, the Health Center will increase patient screenings for opioid addiction. Staff including Behavioral Health clinicians and Internal Medicine providers will be trained to screen, identify, assess, counsel, treat and serve people who have substance use disorders.
· West Rock Care Site: Thanks to a generous $1,000,000 grant from the U.S. Department of Health & Human Services, Health Resources and Services Administration, the Health Center will be establishing a new care site at the Elm City Communities/Housing Authority of New Haven’s Wilmont Crossing Development (122 Wilmot Street) in the West Rock neighborhood. The site will provide primary care, mental health and substance abuse assessment and treatment, and specialty care. The primary care services provided will include pediatrics, internal medicine, gynecology and family planning. Specialty services such as podiatry will be offered on-site part-time. Construction is expected to begin next month.
· Mobile Dental Program: Another recent award of $350,000 from the U.S. Department of Health & Human Services, Health Resources and Services Administration will allow the health center to purchase portable dental equipment and provide mobile dental services. The Health Center plans to provide mobile services at several locations throughout New Haven County including the new care site at the Wilmont Crossing Development.
· Q House Rebirth: The Health Center is proud to be a part of New Haven’s Q House rebirth. The Q House, which played such an important role in the lives of New Haven’s youth for many years, shut its doors in 2003. Now it is being demolished and rebuilt, with a scheduled 2018 opening. The Health Center will be moving its innovative and state-of-the-art medical services, currently located at 226 Dixwell Avenue, to the new Q House when it opens.
· The Joint Commission Reaccreditation: Recently, the Health Center underwent a reaccreditation survey by The Joint Commission, a national accreditation agency. Most of the Health Center’s component programs and sites as well as its Patient-Centered Medical Home status were up for reaccreditation. The Health Center is proud to report that it passed the reaccreditation survey with flying colors. In fact, The Joint Commission noted several areas of best practice and complimented the Health Center on its innovative programs and integrated approach to care.
